Educational Overview
Benign Prostatic Hyperplasia
What is BPH?
Benign Prostatic Hyperplasia (BPH) is a non-cancerous enlargement of the prostate gland.
It is the most common prostate condition in intact (non-desexed) male dogs, especially middle-aged to senior dogs.
The prostate naturally enlarges with age under the influence of testosterone. In BPH, the gland becomes uniformly enlarged, sometimes causing pressure on the rectum and urinary tract.
Why Does It Happen?
BPH is hormone-driven, primarily due to:
-
Long-term exposure to testosterone
-
Age-related hormonal changes
-
Increased sensitivity of prostate tissue to hormones
Neutered males rarely develop BPH.
Common Signs
Symptoms often develop gradually:
-
Straining to defecate (tenesmus)
-
Ribbon-like or flattened stools
-
Constipation
-
Dribbling urine
-
Blood-tinged discharge from the penis
-
Difficulty urinating (less common)
-
Lethargy if secondary infection develops
Some dogs show no obvious signs and the condition is found during routine exam.
Diagnosis
Veterinary evaluation may include:
-
Rectal examination (smooth, symmetrically enlarged gland)
-
Ultrasound imaging
-
Urinalysis
-
Prostatic fluid evaluation
-
Bloodwork (to rule out infection or other disease)
BPH must be differentiated from:
-
Prostatitis (infection)
-
Prostatic cysts
-
Prostatic abscess
-
Prostate cancer
